Find a Provider

Your Partner in Everyday Health

Your primary care provider (PCP) is often your first point of contact for managing your health. From annual checkups to treating common illnesses, primary care is the foundation of long-term wellness. At Highpoint Health Partners, our primary care services focus on prevention, early detection and whole-person care, so you can feel confident about your health at every stage of life.

Services include:

  • Annual wellness exams and checkups
  • Sick visits
  • Preventive screenings
  • Flu shots and other immunizations
  • Minor injuries or accidents
  • Medication management
  • Allergy treatment
  • Healthy lifestyle education
  • Ongoing care for chronic conditions
  • Telehealth now available

Why Primary Care Matters

Primary care is more than just an annual physical. It’s an ongoing relationship with a trusted provider who knows your health history, helps prevent problems before they start and coordinates care when you need a specialist. Having a PCP can lead to better overall health outcomes, fewer hospital visits and earlier diagnosis of conditions.

What Primary Care Covers

Our primary care services include care for people of all ages. Common services include:

  • Preventive screenings and annual wellness visits
  • Vaccinations and immunizations
  • Management of chronic conditions, such as diabetes, high blood pressure and asthma
  • Care for acute illnesses, like infections, colds or flu
  • Routine lab work and imaging referrals
  • Counseling on nutrition, exercise and healthy lifestyle choices

Preventive Care and Wellness

Prevention is one of the most important parts of primary care. By scheduling regular checkups and screenings, you can catch potential problems early, when they are easiest to treat. Your PCP can offer personalized advice to help you stay healthy day to day. This may include guidance on weight management, stress reduction and healthy habits that reduce the risk of disease.

What to Expect at Your Visit

Your primary care visit is a chance to build a relationship with your provider. During a typical visit, your provider may:

  • Review your health history
  • Check your vital signs
  • Order lab tests or screenings if needed
  • Answer your questions and discuss any concerns
  • Provide referrals to specialists when necessary

The goal is to create a care plan that supports your long-term health and fits your lifestyle.

Support for Chronic and Acute Conditions

Primary care providers are trained to manage both ongoing and short-term health concerns. If you have a chronic condition, your provider will help you manage medications, monitor your progress and coordinate care with specialists. For acute illnesses, you can count on timely treatment to help you recover quickly and safely.

Coordinated Care Across Services

Primary care is at the center of your healthcare journey. Your provider works closely with other specialists, such as cardiologists, neurologists and surgeons, when you need advanced care. This ensures all aspects of your health are connected and managed in one place.

Take the Next Step

Whether you need a routine checkup or have a specific concern, Highpoint Health Partners's primary care providers are here to support your health. Taking charge of your wellness starts with choosing the right partner for your care.
